127: Building Healthier Communities: Engaging Local Networks to Address Social Needs.

The Cook Children’s Center for Community Health (C4CH) is dedicated to improving the health of every child in our eight-county service region in North Texas. C4CH oversees community health needs assessment, community research, and community health outreach services. The community health outreach programs utilize a train-the-trainer model to empower community partners to become program champions in customizing programming for both rural and urban environments, ultimately enhancing impact and sustainability. This poster focuses on the Parent Café model, which creates a supportive relationship among caregivers, fostering resilience. Since 2019, C4CH has trained 99 new facilitators, hosted 263 Parent Café sessions, and engaged 1,137 caregivers. After attending a Parent Café, 99% of caregivers reported they learned new ways to deal with family challenges in a healthier way. This approach provides valuable lessons and strategic insights that participants can apply in their respective communities, ultimately contributing to the overall well-being of those communities.
