Measuring Impact at Scale: Evaluating Maternal and Infant Health Pathways in Ohio Using Statewide Longitudinal Data

Ohio continues to grapple with persistent racial and socioeconomic disparities in infant mortality, particularly impacting non-Hispanic Black infants. In response, the state has implemented the Pathways Community HUB model to provide wraparound support to at-risk mothers through community health workers (CHWs) who help families navigate 20 essential service pathways. This paper presents findings from a longitudinal analysis of eight years of statewide data, evaluating the relationship between program participation, timing of enrollment, and birth outcomes. The analysis tracks key outcomes, including preterm birth and low birth weight, across all Ohio HUB sites, highlighting the importance of early and sustained engagement with services. Aligning with AEA’s 2025 theme—which emphasizes evaluation’s role in advancing social equity—this study demonstrates how evaluation can support evidence-based policy decisions that directly address racial health disparities. This work offers a model for using large-scale program data to drive more equitable health outcomes while demonstrating cost-effectiveness.
